There are many ways to obtain the right to healthcare in Spain.
The main and simplest one is being registered with the Spanish Social Security system, either as an employee, self-employed worker, or by receiving any Social Security benefit in Spain.
If you receive a pension from a country in the European Union, the European Economic Area, the United Kingdom, or Switzerland, you can request an S1 form from your country of residence certifying your pensioner status. With this certificate and proof of residence, you can apply for healthcare in Spain.
You can also benefit from the European Health Insurance Card. You may be entitled to it if you receive healthcare coverage from a member state of the European Union, Iceland, Liechtenstein, Norway, Switzerland, or the United Kingdom. This card is mainly used for travel, allowing you to receive medical care in the countries you visit.
If you are not covered by any country’s healthcare system, you can apply for the Special Agreement (Convenio Especial). To do so, you must prove continuous residence in Spain for at least one year. Being registered under this agreement allows you to demonstrate access to healthcare in Spain, which is one of the requirements for obtaining residency. However, this option involves a monthly fee of approximately €60 if you are under 65 years old, and around €157 if you are 65 or older.
